Benefits of Chronic Care Management
Chronic Care Management Case Study
In a 2010 case study by the Journal of Aging Health, over 1,100 patients with a median age of 65 were followed for six months to evaluate the benefits of chronic care management programs. After the study concluded, they found that the results of chronic care management programs were very positive. Patients that received treatment through chronic care management programs saw a decrease in both ER visits and hospitalizations, along with an increase in their overall quality of life.
Additional Results Noted in the Study
Increased Physical Activity
Increased Social Activity
Fewer Health Limitations
Because of this and similar research, national practice guidelines have been created and healthcare providers can now be financially compensated for providing chronic care management services to their patients.
